You should complete the Returning Student Verification (RSV). Initial RSV emails (containing your child's unique 'snapcode') will be sent out in April 2026 to all parents/guardians listed. Please read the below question very carefully.  By answering "yes" to full time Home Education, your child will NOT be enrolling into one of our public schools (including SJVS) as a full time student.  If your child will be on campus to attend full time at their zoned school OR if your home education child will participate in sports, electives or any classes ON CAMPUS, you will need to answer "No" and select the "My child will attend his/her zoned school or full time SJVS program" option. Please do NOT complete this form if you have been awarded or are applying for PEP (Personalized Education Program). Please review the below carefully and confirm your Notice of Intent to Establish and Maintain a Home Education Program (F.S. 1002.41). If you do NOT intend to enroll your child as a full time Home Education student, please review your selection above and select 'no' to continue with enrollment in your zoned school or St. Johns Virtual School. 1. This form will be stamped on the date received in the SJCSD Home Education Office, and receipt will be acknowledged by email. The program start date identified by the parent will be honored unless the form is received more than 30 days later, in which case date of receipt will be used. The annual educational evaluation will be due each year on the month and day of the program establishment. Failure to submit an annual educational evaluation will result in termination of the home education program.

2. A Home Education program established for a student who has been previously identified as exhibiting a pattern of nonattendance will be monitored by a Portfolio Review Committee as require by section. 1003.26(f)(1). Florida Statutes.

3. If the Home Education program ends because the student enrolls in a public school or meets compulsory attendance requirements through a different option, a written notice of termination and the required annual evaluation shall be filed with the Home Education Office within 30 days after said termination.

4. Home education students who access any SJCSD service (e.g. classes, activitiesor testing) will be required to meet the same entry requirements as public school students (e.g. proof of residency, Immunization Form 680, etc.). Students who wish to access classes or activities must register at their zoned school.

5. It is the responsibility of the parent/guardian to submit annual education evaluations and notify the Home Education Office of residential address, email, and telephone number changes.

In accordance with section 1002.41, Florida Statutes, it is my intent to establish and maintain a Home Education Program for the below student. In addition to the information in the notice section, I also understand that the St. Johns County School District is not authorized to issue a Florida high school diploma to home education students.

Parent/Legal Guardian is required to complete an emergency medical form annually for each child.

Please print the Medication Authorization Form, complete it in its entirety, including physician and Parent/Legal Guardian signature and return to the school office/clinic. This form must be on file before medication can be administered at school.

School district personnel will contact Emergency Medical Services directly in an emergency situation and will take whatever action is deemed necessary for the health of the aforesaid child. The school district is not financially responsible for the emergency care and/or transportation for said child.

If your child has Medicaid coverage, and receives additional services during the school day, please consider allowing the school district to seek reimbursement from Medicaid for these services.  Medicaid-reimbursable services include school nurse services, occupational therapy, physical therapy, speech therapy, counseling, and specialized bus transportation. Reimbursement for school-based services is not one of the minimum covered services for managed care plans serving Medicaid enrollees and will not affect your child’s Medicaid benefits.

The Family Educational Rights and Privacy Act (FERPA) affords parents and students over 18 years of age certain rights with respect to the student's education records. The St. Johns County School Board has described Student Directory Information and the conditions for its release in Board Rule 5.20 listed on the District’s website. Please refer to Rule 5.20 for more details. Parents or adult students who object to the release of Directory Information must notify the District and their school annually in writing within 30 days following registration.

Students may receive State specified health services, vision, hearing, weight, BMI and scoliosis screening. Students may be exempted from any of these services if parent or guardian requests such exemption in writing.

Parent/Legal Guardian Statement: I accept responsibility for notifying the school of any changes of home address or phone number or any change in health status of my child. In the event of serious illness or accident and the school cannot contact me, I give permission to have my child moved via ambulance or other conveyance to a hospital for immediate attention, and I assume responsibility for payments of same. In case of an accident or illness when immediate treatment is not needed, but when my child is unable to remain in school, I request to be contacted by the school.
