Please complete the additional questionnaire for NIHF STEM Middle School. A link to the questionnaire will be included in your confirmation email. NIHF STEM Middle School questions MUST be submitted to NIHF STEM MS by 3:00 PM, Thursday, January 15, 2026.

I verify the above information to be correct, and I understand that completion of this form does not guarantee placement in a school. I further understand that I cannot register my child without appropriate address documentation and that only one application can be submitted per child.

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.

In the event my child is accepted, I hereby grant permission for school/education records to an accredited school as needed. My signature further indicates I have read the Open Enrollment Regulations and Guidelines.

I understand that if my child's attendance becomes a concern, their open enrollment may be revoked.

Required Documents

If your child does NOT currently attend an Akron Public School (APS), please provide:

• Student’s Birth Certificate
• Guardian’s Photo ID
• Two current proofs of residency in the guardian’s name
• IEP and MFE/ETR, if applicable
• Custody documents, if applicable

If your child currently attends an APS school:

• Guardian’s Photo ID is required
• Proof of residency is only required if your address has changed

Document Deadline

All required documents must be submitted by December 16, 2026, at 4:30 PM.
Applications with missing required documents after this deadline will not be processed. No exceptions will be made.
