You have my permission to take whatever action is deemed necessary for the health and welfare of my child.

I agree to notify the Athletic Director immediately if my child develops an illness or injury during the season, which would affect his/her ability to participate in any practices or contests.

Injury and Risk Agreement

I understand that such activity involves the potential for injury which is inherent in all sports. Even with the best coaching, use of the most protective equipment, and strict observance of rules, injuries are still a possibility. On rare occasions, these injuries can be so severe as to result in total disability, paralysis or even death.

I give my permission to the appropriate certified school staff or medical personnel to render emergency treatment, if required, when associated with an athletic injury or illness.

I agree not to hold the school or anyone acting on its behalf responsible for any injury occurring to the student in the course of such athletic activities or travel.

Rocky Hill High School Interscholastic Sports Insurance Parent or Guardian Approval

The Board of Education has provided an insurance policy to pay for usual and reasonable medical
expenses (subject to certain policy limits) not covered under your private insurance for injury sustained
while your son/daughter is engaged in interscholastic sports, bands , cheerleaders and drill teams. This
coverage has been designed to provide maximum benefits at a minimum cost. Benefits will be considered
for those eligible expenses, which are left unpaid by other insurance health plans.
A claim form must be submitted within 90 days from the accident date or the claim will be denied. It is the
parent’s/guardian’s responsibility to file the completed claim form within that time period. Forms can be
picked up in the High School’s Main Office or Athletic Department Office. The Accident Claim Form
can also be found on the RHHS Athletic Website under the “Incident/Accident Report Forms” tab.
PLEASE NOTE:
The student accident insurance contains some benefit limits. Therefore, it may not provide 100%
coverage.
Your signature will allow your son/daughter to try out for and/participate in interscholastic sports
activities for Rocky Hill High School with full knowledge of the sports insurance as stated herein.
I/We as parent or guardian of above named student have read and understand the above information
regarding interscholastic insurance coverage. This approval for participation in no way eliminates my
responsibility for medical expenses as covered by any personal insurance policy.
PLEASE NOTE:
Without signature of parent or guardian, the student is forbidden to try out for or participate in the Rocky
Hill High School Athletic Program.

Student Athlete Transporation Guideline

Student-Athlete Transportation To/From Competition:

Students must use the transportation provided by the District to and from games. If there is a need for the
student to use other transportation, the student and parent/guardian must complete the Travel Release
Form which can be obtained in the Athletic Director’s office or provided by a coach. It can found in the
link below and on the Rocky Hill High School Athletic Website. This form must be signed and submitted
to the athletic staff prior to the student using a different form of transportation from an event.

Violations of these policies can result in removal from team.

ROCKY HILL HIGH SCHOOL ELIGIBILITY REQUIREMENTS

Academic Eligibility
To be eligible for participation in Rocky Hill High School Athletics Programs, Student-Athletes must be in good
academic standing and meet the requirements outlined in policy below.
Academic eligibility for fall season is determined by successfully accumulating four credits toward graduation in
the preceding school year.  Failures made up in summer school (or in an approved tutorial) are acceptable.
Eligibility in fall sports is reviewed at the completion of the first quarter; continuation in a fall sport is dependent on
the athlete passing four full time subjects effective the day report cards are issued.
Academic eligibility - competition in the winter season is also based on successful progress (60 or better) in four
full time classes for the first quarter. Continuation in a winter sport after the end of the first semester is dependent on
successful progress during the second quarter, and is based on second quarter grades at the time report cards are
issued, not semester or exam grades.  
Academic eligibility to begin competition in the spring season is based on successful progress (60 or better) in four
full time classes for the second quarter. Continuation in a spring sport after the end of the third quarter is dependent
on successful progress during the third quarter, and is based on those grades at the time report cards are issued.
Students who are ineligible at the start of a season may become eligible during the season. In these cases, an
athlete may join a team whose season is in progress.  For teams that conduct tryouts as a condition of qualifying for
a roster spot, the athlete will be required to meet the same tryout conditions as other team members and may be
subject to cuts.  Prior to participation in any contest, previously ineligible athletes must first practice in a team
setting for a minimum of 10 practices.  At the discretion of the coach, and providing all other conditions for
participation have been met, these 10 practices may occur before the athlete becomes eligible.   In these instances,
the athlete may not exceed 10 practices while ineligible. 
Additional Eligibility Requirements
a. You are nineteen (19) years of age; Student-athletes will be allowed to compete up through their 19th birthday,
however, if their 20th birthday falls during a season, the student-athlete will not be allowed to start or compete
during that season and all eligibility will cease. (Rule II.B.)
b. You have changed schools without a change of legal residence; (Rule II.C.)
c. You have been in attendance for more than eight (8) semesters (A student has eight (8) consecutive semesters or
four (4) consecutive years of eligibility from the date of entry into the ninth grade to be eligible for interscholastic
competition); (Rule II. B.)
d. You play or practice with an outside team in the same sport while a member of the school team after the first
scheduled game in any season; (Rule II.E.) 
The exception to Rule II.E. shall be:
1. Participation in parent/child tournaments and caddy tournaments.
2. Swimming, tennis, gymnastics – a pupil may practice but not compete with a non-CIAC team or
as an individual during the season as long as such practices do not interfere with or replace
member school practices.

e. You play under an assumed name on an outside team; (Rule II.F.)
f. You receive personal economic gain for participation in any CIAC sport. (Rule II.F.)
g. Your work habits or conduct both in and out of school are such as to make you unworthy to represent the ideals,
principles, and standards of your school.
h.  Local rules may be more restrictive than those of the CIAC. (See additional eligibility requirements under
policies and procedures).
i. All students who participate on an interscholastic team must have a physical examination completed by their
private physician each and every year. This consent form, and the State of Connecticut Health Assessment Record,
MUST be completed and handed in to the Athletic Director or Nurses Office, PRIOR TO PARTICIPATION.
Physical examinations for interscholastic athletic participation are valid for 13 months.
*For fall sports eligibility – students must have received credit for four (4) units or its
equivalent towards graduation at the close of the school year preceding the contest.
NOTE: Marking period grades (not semester grades) are to be used in determining
scholastic eligibility, except for fall eligibility.

Rocky Hill High School Student-Athlete Code of Conduct

Rocky Hill High School recognizes that students who participate as members of athletic teams should
conduct themselves as responsible representatives of their school and community. Student-athletes must
always serve as exemplars of high moral character and must demonstrate appropriate academic
commitment, which is expected from all students. As recognized members of Rocky Hill High School,
student-athletes are expected to conduct themselves both on and off the field in a way that exhibits respect
for themselves and others.
Student-Athletes Should:
- Make every effort to develop their own athletic abilities in terms of skills, techniques, tactics, and
stamina.
- Give maximum effort and strive for the best possible performance while practicing or participating in a
game.
- Make every effort consistent with fair play and the rules of the game to help his/her own team be
successful.
- Know and abide by the rules and spirit of the game, and show proper sportsmanship at all times.
- Athletes shall attend and not be tardy for all practices, games, and meets held both during regular school
sessions and during vacations. Exceptions shall be subject to the approval of the head coach.
- Treat opponents with due respect at all times, irrespective of the results of the game.
- Accept the decision of the game officials without protest and demonstrate due respect towards game
officials.
- Set a positive example for others, particularly young players and opponents.
- Refrain from using inappropriate language.
- Athletes shall attend all classes during school day, unless verified as an excused absence by the
Attendance Office. Any non-excused absence from class will result in ineligibility to participate in
practice or competition that day or the next.
- Refrain from using tobacco products, alcoholic beverages, or use of or possession of non-prescribed
controlled substances or paraphernalia
- Always conduct themselves in an exemplary manner in school, the community, and on campus, which
their team is visiting.
Acts of unacceptable behavior/conduct such as, but not limited to, theft, vandalism, arrest,
discrimination based on race, gender, or sexuality, or any violation of the law, will not be tolerated and
can result in disciplinary action and possible dismissal from the athletic team by the coach and/or
school administrator.

I/We have read and understand the student-athlete athletic code of conduct and will to the best of my
ability follow it.

ROCKY HILL HIGH SCHOOL ATHLETIC TRAINER/STAFF CONSENT TO TREAT

I understand that Rocky Hill High School employs an Athletic Trainer to provide sports medicine services
for the school's athletes. I hereby give consent for an athletic trainer and/or a physical therapist physical
therapy or sports medicine clinical staff to provide sports medicine services for the above minor. Sports
medicine services include, but are not limited to: administrating first aid for athletic injuries, providing
initial treatment and management of acute injuries, and assessing athletic injuries at the request of the
athlete, the athlete's coach, or the athlete's parent/ guardian.

I also consent to Rocky Hill High School coaches, nurse or other medical personnel hired by Rocky Hill
High School or Emergency Medical Services personnel called to Rocky Hill High School to provide first
aid and other necessary emergency medical treatment.

I understand that the sports medicine personnel will perform only those services that are within their
training, credential limitations and scope of professional practice to prevent, care for and rehabilitate
athletic injuries. I understand that a written report will be prepared by medical personnel for any athletic
injury assessment for the athlete and will be confidentially maintained in the files of the athletic training
office or school nurse's office. I, hereby authorize the sports medicine personnel who provide services to
the above-named athlete to disclose information about the athlete's injury assessments and post-injury
status. I understand such disclosures will be done, as needed, with the involved coaching staff, Athletic
Director, Principal or other administrators of the school, the school nurse, any treating healthcare provider
and/or consulting concussion management specialist. I understand that there is no charge to me for the
above listed services performed at Rocky Hill.

Uniform/Equipment Care and Return Policy

Uniforms and equipment issued by the school must be returned at the end of each season in
appropriate condition. Equipment or uniforms that are not returned or returned damaged or
stained due to improper care, use, or wear will result in a financial obligation being placed on
student’s account. Students with outstanding obligations are not allowed to participate in extra-
curricular or special school events (including athletics, school dances, and graduation).
Uniforms that are not returned must be replaced for future seasons. Replacement cost of a single
uniform can range anywhere from $50.00-$350.00.
I have read and understand the Uniform/Equipment Care and Return Policy. I agree to the
financial responsibilities associated with my student-athlete participating in an athletic activity in
which they will be issued a uniform or equipment that is required to be returned in appropriate
condition at the end of the athletic season.
