I hereby affirm that the student named on this enrollment form resides with one or both custodial parents/court appointed legal guardians at the address shown and does in fact occupy this residence, and I further declare that this residence is located within the boundaries of the city of Muscle Shoals. I understand the school office should be notified of any address change within two weeks from the time of the move; however, if I move out of city, I am responsible for reporting my new address to the school immediately.
