UPON ACCEPTANCE, we will ask that you provide us with the following Enrollment Documents:

• Copy of certified Birth Certificate
• Proof of Residency (utility bill)
• Current Immunization Record
• Current Transcripts (grades 8-12)
• Home Language Form
• IEP/Eligibility, 504 or GT Documentation (if applicable)
• Custody or other Legal Documentation (if applicable)

Documents are due within 5 days of acceptance to reserve the seat.
Proof of family relationship may also be required for verification.

Kindergarteners must be age five on or before September 1st, 2025. First graders must be six on or before September 1st, 2025.

North Star does not provide a school-sponsored lunch program or a free/reduced lunch program.
