Please read the following completely before you start this application.

**Only one Open Enrollment request form is allowed per student. If you fill out more than one request, all requests will be void and your student will not be eligible to participate in Open Enrollment.

**If your student is not currently attending school in the Campbell Union School District in Transitional Kindergarten - 7th grade, a New Registration must be completed by 4:00p on Friday, February 14, 2025 in order for this Open Enrollment request to be valid. If your student is a new registration, ALL required documents MUST also be turned in by 4p on Friday, February 14, 2025. There will be no exceptions. Please visit https://www.campbellusd.org/files/reg-checklist.pdf to view a list of required documents.

**Completing an Open Enrollment form does not register your student in the Campbell Union School District. If your student is not currently attending school in the Campbell Union School District in Transitional Kindergarten - 7th grade in the current school year, you must complete a 2025-2026 New Student Registration Form found on our website at: https://www.campbellusd.org/enrollment.

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.
