Bridges Preparatory School reserves the right to place students in the appropriate grade level and classes based on records obtained from previous school. Bridges Preparatory School will make placements accordingly.

My signature on this form indicates my decision to have my child attend the charter school below and signifies my request that appropriate school records be forwarded from the school district to the charter school, should my child be selected in the February Lottery Process. My signature also certifies that if my child enrolls in another public school, a nonpublic school or a private school after he or she has been offered a seat in this charter school, I will notify the school immediately to relinquish my seat to the next applicant on the waitlist.
