Required Documentation - Proof of Residence

Massachusetts Charter School admissions guidelines mandate that proof of residency must be provided prior to the admissions lottery. The following documents are acceptable as Proof of Residence; Please provide a copy of ONE of the following with your application:

  • A Utility Bill (not water, cable or cell phone) dated within the past 60 days

  • A Deed, Mortgage Payment dated within the past 60 days, or Property Tax Bill dated within the last year

  • A current Lease, Section 8 Agreement, or Landlord Affidavit (Address Verification form)

  • A W2 form dated within the year or a Payroll Stub dated within the past 60 days

  • A Bank or Credit Card Statement dated within the past 60 days

  • A Letter from an Approved Government Agency* dated within the past 60 days

*Approved government agencies:

  • Departments of Revenue (DOR)
  • Children and Family Services (DCF)
  • Transitional Assistance (DTA)
  • Youth Services (DYS)
  • Social Security
  • Any communications on Commonwealth of Massachusetts Letterhead

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.
