Open Enrollment Interest Form 24-25 2024-2025

This is only an interest form for Open Enrollment.

This is NOT a school enrollment/registration form. Please DO NOT submit a New Student Enrollment Application prior to receiving an open enrollment offer. 

NOTE: This form is to be completed only by the parent/legal guardian.

All Open Enrollment Forms received between December 1, 2023 and January 26, 2024 will be eligible for the priority group classification, per district policy. Forms received after that date will be placed according to the date and time the form is received. Read more about the Open Enrollment Procedures on the district website.

A separate application MUST be completed for each student. 

Student Information

Non-resident student is a student who resides outside of the Madison School District's boundaries who seeks to enroll in the District.

Resident transfer student is a student who resides in the District who seeks to enroll in a school outside of the student's attendance area. 

Currently Enrolled Student is a student that is currently enrolled in a Madison School and is seeking transfer to another school in the District.

ATTENTION

To further process your student's Open Enrollment Interest Form, any prior or current evaluation information that qualifies a student to receive support services (i.e. IEP, OT, PT, Speech, MET, 504 VI, etc.) must be uploaded to this application.

Your student's Open Enrollment Interest Form is considered incomplete unless prior and/or current records are submitted.

Please note that omitting or providing false or inaccurate information on this form may result in the form being denied or admission being revoked.

Open Enrollment acceptance is on a year-by-year basis and subject to re-application and review each year. The parent/guardian signing this interest form affirms that the student seeking enrollment will abide by the rules and regulations that govern students at the school where the student seeks enrollment. Continued open enrollment is based upon compliance with all school regulations regarding conduct, academic progress, and attendance which includes the timely pick-up from school, activities and events. An open enrollment may be revoked at any time for failure to comply with school and district rules (Policy JFB).

By submitting this form, I am stating this is complete and truthful information.  I further understand that by providing false information or omitting information on this form may result in the form being denied or admission being revoked.
