The Lewis Central School district requires proof of residency. Acceptable examples would be a current mortgage/lease agreement or a current utility bill (gas, electricity, water, trash, or cable) for the residence in which the student lives.
If you live outside the Lewis Central School District boundaries you must have Open Enrollment paperwork filed with the school district and approved prior to your enrollment being accepted. You can obtain the form here or call Anna Boehm at 712-366-8248 if you have any questions.

Immunization Requirements & Exemptions (per Iowa HF 299) Iowa law requires that all children enrolling in school or licensed child-care programs are up to date on required immunizations. However, exemptions are allowed in the following cases:

All kindergarten children newly enrolling in an Iowa elementary or ninth grade students in an Iowa high school are required to have a dental screening. This requirement was passed by the 2007 legislature and became effective July 1, 2008. The purpose of the dental screening requirement is to improve the oral health of Iowa's children. Dental screenings help with early detection and treatment of dental disease; promote the importance of oral health for school readiness and learning; and contribute to statewide surveillance of oral health. The requirement applies to kindergarten and ninth grade students only.

Beginning in 2008, all enrolling kindergarten children are required to submit proof of lead testing. This test is usually performed between the ages of 12-18 months of age at a medical well- child check. Your child’s medical provider will confirm proof the test was completed on the Kindergarten physical examination form.
Medical and religious exemptions are available on the Iowa Department of Public Health Mandatory Blood Lead Testing Program website. If used they must be notarized and returned to Kreft. (Here is the link for the IDPH website: Mandatory Blood Lead Testing Program )
Beginning with the 2015 school year, Iowa law now requires vision screening for all kindergarten and 3rd grade students. Iowa Department of Public Health conducts an annual audit for compliance. The vision screening form may be completed by your child’s medical provider or an eye care professional and returned to your child’s school.
