Please enter the student name as it appears on the birth certificate, passport or other official document.

If you are a currently approved AAPS Schools of Choice or Non Resident Child of Employee student applying for an In-District transfer, please select yes that you reside in the AAPS area.

Enter the Michigan address at which the student lives (or will live) the majority of the time. If student splits time between parents, please enter one location to be used as the primary address during the pre-enrollment process. An opportunity to enter a second address will be offered during the enrollment process.

I verify the above information to be correct, and I understand that completion of this form does not guarantee placement in a school. I further understand that I cannot register my child without appropriate address documentation and that only one application can be submitted per child.

I understand that a child admitted under false information is illegally enrolled and could result in disenrollment upon discovery. I further understand that it is my responsibility as the Parent/Legal Guardian to immediately inform the district of any changes to the information provided.
